Health Maintenance Organization. Lower costs, must have a PCP, smaller and more restrictive network.
PPO
Preferred Provider Organization. Higher costs, does not require a PCP, less restrictive network
C-SNP’s
Chronic Conditions Special Needs Plans. Only available to individuals with specific severe or disabling chronic conditions, SNP’s cover the same Medicare services that all Medicare Advantage plans must cover, may also cover extra services tailored to the special groups they serve, like extra days in the hospital.
D-SNP’s
Dual Eligible Special Needs Plans. Only available to those who qualify for both Medicaid and Medicare, D-SNP’s cover eligible doctor visits, hospital stays and prescription drugs.
