Facilitation
The use of posture, action and words to encourage the patient to continue to say more without specifying the topic.
Reflection
A repetition of the patient’s words that encourage them to continue.
Clarification
Asking the patient to explain unclear, or ambiguous words.
Empathetic Responses
Respond in a way to show understanding, and acceptance of the patient’s feelings.
Confrontation
Used to point out inconsistencies in the patient’s story or between their story and their actions.
Interpretation
Make an inference, rather than an observation.
Asking About Feelings
When you sense important, but unexpressed feelings, inquire.
Chief complaint
The reason, in the patient’s own words, why care is sought.
History of the Present Illness
Full, clear, chronological account that amplifies the chief complaint
Tinnitus
Ringing sensation in the ears
Vertigo
A type of dizziness that has a motion sense. Often the patient feels that the room is spinning.
Palpitations
An unusual awareness of one’s heartbeat. Often described as “racing heartbeat,” skipping, or fluttering.
Orthopnea
Difficulty in breathing associated with positional changes, for example, increased difficulty when supine.
Melena
Dark, tarry stool, indicating digested blood from upper gastrointestinal bleeding.
Hematochezia
Passing bright red blood per rectum.
