Chief Complaint or Complains Of: patient’s description of what brought him or her to the doctor or hospital; usually brief and often documented in the patient’s own words indicated within quotes
HPI
History of Present Illness: expansion of chief complaint including details of duration and severity of condition

Sign: objective evidence of disease perceptible to the examiner (ex. elevated blood glucose)
