Macule
flat, area of skin that is different in color; <1cm. (ex. freckle, mole/nevus)
Patch
flat, macule >1cm diameter (port-wine stain, cafe au lait spots)
Papule
elevated, firm, round area <1cm diameter (ex. wart, elevated mole/nevus)
Plaque
elevated, firm, rough lesion with flat top surface >1cm diameter
Wheal
elevated, irregular shaped cutaneous edema, solid, transient (ex. insect bite, allergic reaction)
Nodule
elevated, firm, round lesion, deeper in dermis than a papule, 1-2 cm diameter (lipoma)
Vesicle
elevated, round, superficial, not into dermis, serous fluid sac, <1cm diameter (ex. chicken pox)
Bulla
Vesicle >1cm diameter (blister)
Erosion
loss of part of the dermis, depressed, moist, glistening often after a vesicle/bulla ruptures.
Pustule
elevated, superficial lesion, purulent fluid filled sac (ex. acne)
Cyst
elevated, round, encapsulated lesion, filled with liquid or semisolid material (ex. sebaceous cyst)
Scale
heaped up, keratinized cells, flaky skin, irregular, thick/thin, dry/oily
Lichenification
rough, thickened epidermis secondary to persistent rubbing, itching, or irritation. Often over flexor surfaces (chronic dermatitis, skin inflammation)
Plaque (scaly)
patch of raised cells (scale), usually a silver appearance (ex. Psoriasis)
Telangiectasia
fine, irregular red lines form capillaries (ex. rosacea or “spider veins”)
Excoriation
loss of epidermis (abrasion or scratch)
Fissure
linear crack or break from the epidermis to the dermis. May be moist or dry (athletes foot). Treated with anti fungal agent.
Scar
thick/thin fibrous tissue that replaces normal skin following an injury.
Keloid
irregular-shaped, elevated, progressively enlarged scar, grows beyond boundaries of wound.
Tumor
elevated solid lesion; may be clearly demarcated, deeper in dermis. >2cm diameter (ex. neoplasm, hemangioma)
Pruritis
itchy skin
Eczema/dermatitis
inflammatory disorder of the skin caused by endogenous or exogenous agents.
