2 patient identifiers and check for allergy

Check the order and gather materials needed

Note skin integrity (intact?), muscle size
Avoid bruised, inflamed, and edematous areas
Palpate

Hand hygiene

Compare MAR to order (1st check)
10.
Pull meds and compare them to the MAR.
Scan PT bracelet and then scan meds (2nd check)
11.
Draw up meds and compare syringe dose to MAR at bedside (3rd check)
Review all 5 safety rights

Perform hand hygiene

Clean the site with an antiseptic swab (center and circle ~2” outward)
14.
Choose needle. Remove cap
15.
Administer med!
SQ-5/8”needle; pinch an inch; stomach, inner thigh, and back of arm
IM-1-1.5”needle; z-track; deltoid, ventrogluteal muscle of the hip, or vastus lateralis muscle of the thigh
ID- 5/8”needle; pull skin aside; inner forearm at 5-15 degree angle
IV push- flush, push, flush

Discard needle & syringe into sharps container

Remove gloves

Reassessment
Did the med work? How are you feeling (look for allergic reaction)?

DOCUMENT IN EHR!!
Med, route, site, dose, time, patient education, patient response, side effects, & observed assessment

Administer through different site or clarify with clinical pharmacist
