Decadron
dexamethasone Na PO4 Corticosteroid Various inflammatory conditions. 2-24mg IV/IO over 1-2 mins. Avoid with concurrent use of ASA
Activated Charcoal
Antidote
Oral poisoning/OD.
50-100gm/dose, given orally
Do not give with ipecac. NOT with methanol, cyanide or organophsophate poisoning, ingestion of caustics.
Dextrose 50% in Water
D50W
Antihypoglycemic
25g SLOW (10cc/min) IV push into large bore DO NOT INFILTRATE
Do not use with increased ICP or IC hemorrhage.
Carefully monitor for infiltration
Valium
diazepam
Benzodiazepine - Skeletal muscle relaxer, acute stress, amnesia.
2-4mg IV/IO slow push, 1-2 minutes
Do not use w/ narrow angle glaucoma, >6mo, allergies.
Expect ataxia, blurred vision, confusion, headache, dec. resp.

diphenhydramine HCl
Antihistamine - Use for allergic reactions, anaphylaxis, mild sedative
25-50mg IV/IO/IM over 1-2min. Do not exceed 50mg single dose
Do not use with asthma, pregnancy, nursing, alcohol abuse, glaucoma, MAO inhibitors.
Inapsine
droperidol
Antiemetic/acute psychosis.
1.25-2.5mg IV/IO, undiluted over 1-2minutes.
Do not use with known or suspected QT prolongation.
May cause chills, dizziness/hypotension, restlessness, tachycardia
Sublimaze

Narcotic analgesic
25-100mcg IV/IO/IM/IN over 1-2min.
Do not use with resp. depression, use caution with TBI
May cause resp. depression, hypotension, sedation, sinus brady.
Mazicon or Romazicon
flumazenil
Benzo receptor antagonist
0.2mg IV/IO over 30sec. Max of 1mg
Do not use with increased IC, antidepressant OD, labor.
Only give if positive an OD on benzo has occurred.
Glucagon
Antihypoglycemic
0.5-1mg IV/IO/IM for hypoglycemia, 2-3mg IV for beta blocker OD
Use caution with pts lactating, renal/hepatic failure, emaciated, alcoholics
May cause nausea, vomiting, resp. distress, urticaria, hypotension
Haldol
haloperidol
Antipsychotic, neuroleptic
2-5mg IM
Do not use with coma, >3yrs, alcohol and barb w/drawal states
May cause EPS, TD, drowsiness, confusion, HTN, EKG changes
Dilaudid
hydromorphone HCl Opiate analgesic Moderate to severe pain 0.5-2mg IV/IO titrated to pain relief Do not use with resp. problems, labor, abd trauma.
Insulin
Hormone (natural and synthetic)
5-20 units IV, SubQ, or IM followed by 0.1unit/kg/hr infusion
Do not use with hypoglycemia
Use with caution with corticosteroids, dobutamine, epi and thiazides
Toradol
ketorolac tromethamine
Short term pain mgmt.
15-30mg IV/IO over 1-2 mins. 30-60mg IM. Equal to 800mg Ibuprofen
Do not use with peptic ulcer, pregnancy or nursing mother, children
May cause drowsiness, dizziness, headache, sweating, edema
Ativan

Benzodiazepam
1-2mg IV slowly, up to 4mg for seizures.
Do not use with COPD, sleep apnea, shock, coma, glaucoma
May cause headache, drowsiness, ataxia, dizziness, resp. depression
magnesium sulfate
MgSO4
Anticonvulsant, electrolyte
1-2mg IV/IO SLOW push, MUST BE DILUTED TO 20% PRIOR TO IV
Do not use with heart blocks, renal disease, recent MI
Make sure to have CaCl or calcium gluconate (antidotes) on hand
Osmitrol
mannitol
Osmotic diuretic
300-500mg IV drip ONLY. 0.5-2g/kg. Not prehospital
Do not use with anuria, CHF, pulmonary edema, preg, dehydration.
Expect headache, nausea, thirst, chills, pulm. edema, acidosis
Demerol

Opiod analgesic
25-100mg IV/IO q3-4hrs PRN
Do not use with COPD, asthma, kidney disease, glaucoma, hypoTN.
Monitor closely as pt may require assistance with resp.
Solu-Medrol

125-250mg slow IV/IO push over 1-2mins
Do not use with systemic fungal infections
May cause headache, HTN. Should not receive immunizations

0.3mg/kg IV over 30 seconds. IV/IO
Do not use in coma, shock, EtOH intoxication, cherry allergies
Monitor closely as it may cause resp. depression
morphine sulfate
Opiod analgesic
2-10mg IV/IO, 5-15mg IM
Do not use with DLOC b/c of head injury, asthma/COPD, hypotension
May cause hypotension, parasymp. increase, bradycardia
Monitor closely, have naloxone ready
Narcan

Opoid antagonist
0.4-2mg IV/IO slow over 15 sec. May go up to 10mg.
Use caution and titrate up. Pt’s may become violent
will cause acute withdrawal symptoms in an addict
Nitrox/N2O2/Entonox
nitrous oxide
Inhalation anesthetic
Given as a 50/50 mix w/O2.
Do not use with any ALOC b/c of head injury, COPD, Pul. Edema.
May cause lightheadedness, drowsiness, nausea/vomiting
Zofran
ondansetron
Antiemetic
4-8mg IV/IO over 1-2 mins
May cause agitation, arrhythmias, CP, drowsiness, dizziness.
Dilantin
phenytoin
Anticonvulsant
Not given pre-hospital. 15-20mg/kg IV/IO.
Do not use if bradycardic, liver disease
May cause dizziness, ataxia, hypotension, confusion, vfib
