Oral Considerations/ Assessment
-Ability to sit up right
-Ability to Swallow
-Look in mouth
-Abdominal distention, nausea, vomiting
-Pain
Oral administration
-Give in med cup
-Offer water
-Stay with pt until they swallow
Buccal and sublingual Considerations/ Assessment
-integrity of oral mucosa
-any signs of lesions, discoloration, masses, skin breakdown
Buccal administration
-Place tablet or film between gums and the cheek.
-Medication will dissolve, do not chew
Sublingual administration
-Place tablet or film under tongue
-Medication will dissolve, do not chew
Topical considerations/ assessment
-Skin condition for irritation, pruritus, burning, lesions, breakdown, rashes
-Ask if patient has ever had a reaction to creams or lotions
-Remove old dose with soap and water before applying new
-Rotate sides
-Follow hair growth
-Date/time/ initial patch
Vaginal considerations/ assessment
-Ask pt if they are experiencing any burning, pruritus, or discomfort
-Have patient in supine with legs abducted
-inspect condition of external genitalize and vaginal canal
-Lubricate
-Insert fingers length
-Remain on back for period of time
-offer perineal pad when patient gets up for ambulation after
Otic considerations/ assessment
-Assess external ear structures and canals for signs of redness, drainage, irritation.
-Ask about hearing loss or ringing in the ears
-Side lying position or sit in chair with head tilted
-If cercumen or drainage present clean with Qtip
-remain in position for 5 minutes following dosage
-Gently massage tragus
-Ask pt if there was any pain during instillation of drops
Rectal
-Look at WBC/PT/PTT/INR due to increased risk of bleeding
-Assess rectal tone/ external anus
-Ask if they can control bowels
-Lay in left ride-lying sims position with right knee bent up towards chest
-lubricate
-have pt take deep breath in through mouth
-insert 1 fingers length and keep inserted for several seconds
-remain flat or on left side for med to absorb
