What does COSHH stand for?

What does LOLER stand for?

What are the requirements of LOLER?
Strong and stable enough for the particular use and marked to indicate safe working loads.
Positioned & installed to minimise any risks.
Used safely i.e. the work is planned, organised and performed by competent people.
Subject to ongoing through examination & where appropriate inspection by competent people.
What PPE must be worn when slinging?

What precautions must be taken when lifting?

What does RIDDOR stand for?

Define a confined space?
A confined space is defined as being an enclosed compartment or space which has remained shut & unventilated for more than 24 hours.
What does a gas monitor test for?
Hydrogen sulphide
Methane
Oxygen levels
List requirements when accessing confined space?
Make sure it’s cool
Force ventilate for 24 hours
Test after 24 hours
Force ventilate for a further 12 hours
Test and repeat if necessary every 12 hours until safe
A gas free certificate must be issued before entry is allowed.
What PPE is required in a confined space?
Overalls
Boots
Barrier cream
Paper coveralls
What precautions are to be taken in a confined space?
No smoking Safety sentry in place Communications between sentry and personnel Sentry must be able to summon assistance Sentry has BA Personnel to wear a lifeline A gas monitor

On completion of welding what will happen next?
Welding sentry must remain for 20 minutes
After 30 minutes sentry performs a set of rounds and informs OOW/OOD & DMESR.
The DMESR carries out rounds in the area at the end of the day.

What is loto?
Loto is a method of safely isolating systems and equipment electrically and mechanically
What are the four main reasons for conducting loto?
The isolation of equipment to allow maintenance to proceed without hazard to the maintainer of the equipment
Where equipment is defective and may be dangerous
Prevention of selection of a mode which may be unavailable or dangerous
Where supplies have been provided but equipment has yet to be fitted
What are the seven considerations which must be conducted as part of the loto process?
All normal and alternative isolation points have been identified
Obtain permission from authorising technical rate
Equipment user and other personnel that may be affected are informed.
A separate loto form is to be filled in for each individual isolation point.
The next sequential number is to be identified from the loto log index
All details to be completed in front of the loto log
All the loto information is to be recorded on a loto safety certificate
