Maverick Modules (20 - 22) Ultrasound FNB, ACB, PPB Flashcards
What depth is used for an ultrasound guided femoral nerve block?
3-5 cm
What “triangle” is looked for to located the femoral nerve?
Fascial Iliaca “Triangle”.
What kind of neeld approach should you use with the FNB?
In plane approach, lateral to medial
Why are two “pops” felt when performing an ultrasound guided FNB?
“pops” occur from needle passing through fascia lata and fascia iliaca
Where do you inject for an ultrasound-only guided Femoral Nerve Block?
Three injections at each corner of the triangle
What was the purpose of the development of the adductor canal block?
Preserve Quad strength post-surgery
What muscles border the adductor canal?
- Sartorius
- Adductor Longus
- Vastus Medialis
What nerve is not frequently found in the adductor canal and requires blockade separate from your adductor canal injection?
Vastus Medialis Nerve
Can the Vastus Medialis Nerve be located via nerve stimulation?
If so, what muscle twitches?
Yes; twitching of medial head of quadriceps will be noted.
Where does the majority of quadriceps strength come from?
- Rectus Femoris
- Vastus Lateralis
What nerve roots does the femoral nerve arise from?
L2, L3, and L4
What is the sensory-only component of the femoral nerve distribution?
Saphenous Nerve
What is the proper probe placement for adductor canal block?
Mid-Thigh, with short axis view of sartorius, vastus medialias, and adductor longus and magnus muscles.
Where is the popliteal plexus block performed?
At the most distal portion of the adductor canal (3cm proximal to the medial femoral condyle)
What makes up the popliteal plexus?
Distal Adductor canal: We block the genicular branches of the posterior obturator N, and the tibial sciatic N.
