Maternity Flashcards
What is RDS?
Respiratory distress syndrome
Not able to breath properly
Babies and risk for RDS
Preterm
Infants of diabetic mom
Decrease blood perfusion to lungs
Signs and symptoms of RDS
Low body temp
Nasal flaring
Tachypnea
Cyanotic mucous membranes
S/S Increased distress in RDS
Grunting Seesaw respirations Decrease trine output Apnea Bradycardia Pneumothorax
How to treat RDS
Surfactant forms at 34 weeks
What do you need to know about admin O2
Used to correct low P02 and pH levels following surfactant admin
CPAP and PEEP apply pressure on alveoli to prevent collapse
Bronchopulmonary dysphasia or
Retinopathy of prematurity leads to blindness
What is transient tachypnea of the new born
At birth 80rmp when crying, within an hour goes to 30-60
TTN= breaths do not slow, 80-120
What causes Transient tachypnea of the newborn
retained lung fluid which limits amount of surface for oxygen exchange or decrease production of surfactant
Onset: 6hrs
Duration: 72hrs
What is meconium aspiration ?
Is baby poop in baby lungs
This occurs from HYPOXIA, that affects vagus nerve to relax rectal sphincter
What is subjective data of meconium aspiration?
Green to black amniotic fluid
Respiratory distress
Number one issue from meconium aspiration syndrome
Inflammation of bronchioles, decrease surfactant due to lung trauma
Secondary infection is pneumonia
What is hemolytic disease of new born (hyperbilbrubinemia)
The fetus has a different blood type then the mother, mother builds up antibodies against the fetal red blood cells leading to hemolysis
What causes hyperbilibrubinemia?
Rh incompatibility
-red blood cells enter maternal circulation
-mom makes antibodies against fetal antigens
RHOGAM is given in this case
What is hydrops fetalis?
Occurs when blood in vascularure becomes hypotonic to interstitial fluid due to decreased RBC causing fluid shift and extreme edema
What are normal levels and high levels of bilirubin. Why is it dangerous to have high levels? What do we do?
Brain damage occurs Body is forced to use glucose stores to maintain metabolism of anemia which equals hypoglycaemia Norm: 0.3mh/100mL Dangerous: 12mg/dl Phototherapy
What are the effects of cocaine?
Vasoconstriction (respiratory and cardiac problems)
Infants suffer hemorrhage, tremors, irritability and muscle rigidity
What does amphetamines do?
Can cause high contractions in maternal circulation
Jitteriness, poor feeding and growth restriction
What does phencyclidine do?
Used for IV anesthetic
Cause long term hallucinations and irritation
Narcotic agents
Euphoria and sedation
PIH, phlebitis, subacute bacterial endocarditis, Hep B and HIV
With draw symptoms: N&V, diarrhea, abdominal pain, shivering, body aches, muscle jerks and insomnia
What is necrotizing Enterocolitis?
Intestinal dysfunction where the bowel develops necrotic patches that interfere with digestion. This can lead to paralytic ileus, perforation and peritonitis
