Scapular Abduction
Tests serratus anterior
Pt: sits w/ shoulder flexed and protracted
OT: hold arm just above elbow, stabilize scapula w/ other hand and push inward and down
5-full resistance
4 slight yield
3- full ROM
2- hold arm up (looking for winging)
1- palpate
0- no contraction
Scapular elevation
Tests upper trapezius and levator scap
PT- sitting- ask to raise shoulders and push down
2,1,0 - prone for ROM and palpattion
Scapular adduction
testing trapezius and rhomboids
Pt- prone on table, abduct shoulder (90 deg)
ask pt to raise their arm to the ceiling as high as possible
apply pressure downward into protraction
Scapular Depression /adduction
low/middle traps
Pt prone on table- abduct shoulder to 90 deg (forearm neutral, thumb pointed to the ceiling)
Ask patient to raise arm as far as possible
***Apply pressure just above the wrist, stabilize below the spine of scapula
5- full
4- apply pressure @ distal humerus instead of wrist
Scapular Adduction and Downward rotation
rhomboids
Pt- prone, arm internally rotation across back, elbow flexed (put hand behind back)
***Apply pressure downward and outward
Grade 2- sitting- put arm behind back, try and move it away
Latissimus dorsi
extension, adduction, medial rotation
Pt: prone w/ head turned to test side, arms at side (shoulder is inwardly rotated (palm up)
Ask pt to move arm up and in - apply resistance on medial forearm (abduction and slight flexion)
Shoulder flexion
(anterior delt, RC, p major, corachobrachialis)
Pt sitting w/ arms at side in slight elbow flexion, forearm prone
Apply pressure just above the elbow, stabilize the shoulder
2- side lying for ROM ask patient to bring arm up
Shoulder Extension
post deltoid, lats, teres major, LH triceps
PT: prone, arms at side, palms up
Pressure just abive the elbow
2- supine- sliding arm out- gravity eliminated
