Q

Standard Pads:

  • Location options
  • Disadvantages
  • ECG needs
A

Location pairings: Anterior/posterior or Right anterior/Apex-lateral.
Disadvantages:
1. Lower quality r-waves than ECG leads
2. No compression detector.
ECG Needs: Need separate ECG leads for pacing/cardioversion.

Q

Pads with a Compression Sensor 4-way feedback

A
  1. See thru CPR rhythm during compressions
  2. Prompts you to go faster if rate<80
  3. Prompts if idle time too long
  4. Depth detector. Hexagon on monitor fills with depth of compressions. (2-2.4)

Q

CPR pad limitation

Should we get ECG leads?

A

less effective R-wave.

Yes use ECG if we need to pace/cardiovert.

Q

How to use One Step CPR pads for atrial cardioversion.

A

invert left anterior pad and place on right anterior so compression sensor still aligns with sternum.

Q

One Step Pacing Pad:

  • Placement
  • ECG leads needed?
A
  • Posterior/ Left Anterior

- No additional ECG leads needed as long as anterior pad is left of sternum.

14
Q

Can One Step Pacing pad be used for atrial cardioversion?

A

Yes, invert the anterior pad and put it on the right upper but R-wave is useless so Yo DO NEED ECG cable.

16
Q

What 4 things are available with a One Step COMPLETE pad?

A

CPR compression sensing
Defibrillation
Cardioversion w/out separate ECG cable
Pacing w/out separate ECG cable

17
Q

Can complete pad be used on R-anterior? Any lost capabilities?

A

Yes, like others, just invert the Left anterior pad and place it on R anterior. In order to detect R-waves, need separate ECG cable.
Can still sense compressions and defibrillate.

18
Q

Placement of One Step Basic Pad?

A

Posterior/L-anterior

Needs separate ECG cable.

19
Q

When do I plug in the One Step ECG adaptor cable to back of defibrillator?

A

Only plug in the One Step ECG adapter if monitoring ECG thru One Step Cable.

20
Q

When must One Step ECG adapter be unplugged?

A

When using standard ECG leads. They must be plugged into the back of the monitor.

21
Q

Which pads have a built in 3 lead ECG?

A

One Step Complete and One Step Pacing Pads.

22
Q

Which One Step Pads DO NOT have Built in 3-lead ECG?

A

One Step Basic and CPR

23
Q

Internal Handles are used for which area of the body?

A

Open hearts when they fibrillate coming off pump.

24
Q

What is the energy dose for internal handles on open heart?

A

10-20 Joules

25
CPR box on screen shows adequacy of chest compressions thru these 3 qualities:
1. Rate prompt 2. Depth Prompt 3. Idle Time
26
Manual Defibrillator Controls (4)
Analyze Energy Select Charge Shock
27
The bottom screen will always show an energy to be delivered UNLESS...
The Control knob is set to Monitor
28
Battery Indicator Lights
Green=good Yellow=charging Alt green/yellow=problems
29
Recorder button does what?
prints out ECG strip
30
How to Defibrillate (5 steps)
1. Place Pad 2. Turn know to Defib 3. Select Energy (200J) 4. Charge 5. Shock
31
If Defibrillating with paddles, name 3 functions performed on paddles themselves:
1. Choose energy 2. Charge 3. Shock
32
How to perform Synchronized Cardioversion: 6 steps
1. Place pads: posterior/L-ant for Vent. Posterior/R-ant for atrial. 2. Turn knob to defib 3. Press "sync" button before each shock attempt. Markers appear over R waves. 4. Select energy: 75-120J 5. Hit charge button 6. Hit Shock button
33
How to perform Transcutaneous Pacing: 5 steps
1. Place Pads: Posterior/L-anterior 2. Turn know to Pace 3. Select HR w/Rate knob. 4. Change energy with mA knob. 5. Set maintenance threshold 10% above pacing threshold. Ex) if capture is at 50mA, set maint. to 55mA
34
What does 4:1 button do?
It suppresses 3 of 4 pacer impulses so I can see intrinsic rhythm to determine if bradycardia remains.
