Managing risk of prescribing Flashcards
Rapid reaction
Rapid drug administration - Red man syndrome following vancomycin
Early reaction
occur early in treatment and then tolerance - like Nitrate-induced headaches
First dose reactions
occur after first dose - hypotension with ACE inhibitors
Intermediate reactions
occur after some delay = longer the delay the less likely - scuh as some alletrgic reactions
Late reations
Risk of ADR increases with continued or repeated exposure - also includes withdrawal reactions - withdrawal from corticosteroids
DElayed reactions
occur after exposure, even if drug withdrawn - teratogenicity for example
Susceptibilities mnemonic
IGASPED
Immunological reactions, Genetics, Age, Sec, Physiology, Exogenous, Disease states
reaction to drug - how long observe?
6-12 hours
Drugs cross-react with penicillins
Cephalosporins - 3rd gen just use with caution
Carbapenems
Monobactams except aztreonam
Anaphylaxis adrenaline dose
<6 150 micrograms (0.15 ml 1:1000)
5-12 300 micrograms (0.3 ml 1:1000)
>12 500 micrograms (0.5 ml 1:1000)
ALL IM - CANNOT GIVE THIS DOSE IV - 1:1000 = 1 mg/ml
Following adrenaline give what?
Chlorphenamine (antihistamine) and hydrocortisone
Chlorphenamine dose?
< 6 months - 250 micrograms/kg
6 month - 6 years - 2.5 mg
6-12 5mg
>12 years 10 mg
Anaphylaxis hydrocortisone dose
<6 months 25 mg
6 month - 6 years 50 mg
6-12 100 mg
>12 200 mg
