Who are the members of the healthcare team?
anyone who is involved with the patient
primary care

ex. primary care physician, vaccines
secondary care
focus on diagnoses and treatment of disease
ex. mammogram, regular health screenings
tertiary care
restoration of health after treatment
ex. physical therapy
health maintenance organizations (HMOs)
more restrictive
requires patient to select primary care physician who manages secondary and tertiary services
preferred provider organizations (PPOs)
less restrictive
cost more, but more flexibility
case management framework
plan of care isn’t linear, lots of jumping around
ex. abused children need this- they need therapy, pharmacy, and regular Dr visits

holistic approach
healthcare organized around expressed physical, and emotional needs of patient

nurse is assigned to a task not patients
ex. nurse is assigned to check the blood sugar of all the patients on a unit
team nursing
nurse is assigned to group of patients to take care of with help of others- what normally occurs on a unit
advocacy
anything we do to help the patient get the care they need

giving tasks to capable people so patients get higher quality care
you are still held responsible
collaboration
work with others to benefit the patient

- health literacy- if a patient doesn’t understand instructions, they won’t follow them
- changing demographics- language barriers
- advances in technology
- equitable access to healthcare
equality vs equity
equality gives everyone the same thing; equity gives everyone whatever they need to succeed
