Management of heart failure - stage 2 + Flashcards
1
Q
What is the 3rd line management of HF?
A
- Ace inhibitors/ ARB and 2. Beta blocker and 3. Aldosterone antagonist
2
Q
Name two side effects of potassium sparing diuretics?
A
- Gynaocomastia 2. Renal dysfunction 3. Hyperkalaemia
3
Q
What is the 4th line management of HF?
A
ACE inhibitors or ARB - ADD diuretic - ADD beta-blocker - ADD aldosterone antagonist - ADD digoxin
4
Q
What is the role of Digoxin in HF?
A
- Sinus rhythm patients that remain symptomatic despite other interventions 2. Used to treat AF in heart failure
5
Q
What should be given in a patient with HF with an arrythmia?
A
Amiodorone
6
Q
What is the mechanism of action of Digoxin?
A
- cardiac myocytes increase uptake of calcium to increase power and contractility of the muscle
- eliminate K+ from cardiac myocytes to increase time for repolarisation thus slow down the heart rate
- inc stroke vol + reduces work of heart
7
Q
Name 2 symptoms of Digoxin toxicity?
A
- Colour blindness
- Tachycardia
antidote - digibind
8
Q
Name 3 surgical interventions in Heart failure?
A
- Valvular replacement
- Implanted Automatic cardiodefibrillator or pacemaker
- Heart transplant may be considered in the end stages
9
Q
Name two implantable devices in Heart failure?
A
- Left ventricular assist device
- ICD - cardiac defibrillator recommended in patients with ejection fraction of <35 %
10
Q
Name 5 adjunct to heart failure?
A
- Opiods - peripheral vasodilation and pain relief
- Statin - reduces cholesterol
- Asprin - inhibits thromboxane and reduces tendency to clot
- Warfarin - patients who have AF
- Amlodapine - calcium channel blocker to reduce blood pressure if still high
