management and delegation Flashcards
What is delegation?
designating tasks to others
Who retains responsibility when designating tasks to others?
the RN
it is the RN’s responsibility to___ the delegated task.
evaluate (to see if it’s done properly and if the patient is all right).
Don’t assume someone is ___ to do something, just because of their job description.
competent (just because someone is an LPN or CNA, don’t assume they know how to do all LPN or CNA things)
you’ve got a CNA that has 20 years of experience. has been working in the well baby nursery for the past 5 years. this person has just gotten pulled to the orthopedic floor & you have a post op hip replacement patient that needs to get up and to the chair.
- CNA’s have been taught basic transfer and positioning of patients. it doesn’t matter that she has 20 years of experience, but her skills are better at her own unit (the well baby nursery… not ortho)
- there are concrete instructions that CNAs have to know when moving an ortho patient. you need to give them specific instructions, or just go to help the CNA with the transfer.
when someone is pulled to a new floor, pretend they are a___ nurse all over again.
- new ; do not give this nurse any patient requiring specialized care
an E.R nurse with 15 years of experience and she gets pulled to the labor & delivery unit. which patient would you give this nurse?
a) severe DVT with bilateral involvement
b) patient in active labor
c) patient who is 8 hrs post-partum
d) patient requiring fetal monitoring
A) any new nurse can care for this patient.
UAP (unlicensed assisted personnelle) also means___
- CNA
- Phlebotomist
- EKG teck
- Lab assistant
(anyone without a license. you’re responsible for anyone who goes in and out of your patient’s room)
when someone does something you did not delegate, you better___
teach, teach, teach… and document that you taught.
when you set a patient’s goal, can the LPN evaluate the patient to determine if the goal has been met?
no! because the LPN cannot do any form of evaluation, since evaluation involves assessment and we never delegate any assessment of any form
who is suppose to all admission history?
RN
always consider a new admit as___
unstable; only the RN will do this
if someone else or the LPN completes the admission data, never sign off on the form until you___
validate the data
can an LPN devise a teaching plan for newly diagnosed diabetics?
no! only standardized teaching plans (basic things… things that are the same for every patients in the world).
it is your responsibility to figure out the staff’s strength and weaknesses. when you figure it out, you’re suppose to__
teach teach teach (As a supervisor)
3 rules of delegation:
1- adequate information
2- give timeframe
3- prioritize (tell them what you want done first)
delegation is not an___ to another RN
assignment
3 rules for delegating to LPNs
1- only nursing care in stable situations
2- orders not subject to change
3- selective tasks in unstable situations (if a code happens, you don’t want lpn to leave… you’ll give them specific things to do)
the more chronic an illness is, the more__ it is
stable (COPD, diabetes..etc)
rules for delegating to nursing assistants:
1- non complex tasks like I & O, routine vital signs,
2- basic communication (they can do cpr too)
3- post mortem care
4- identify risks (patient getting out of bed)
5- teach them to get back to you on the condition of the patient and task you gave them to do
YOU DO NOT LET NURSING ASSISTANCE TAKE VITAL SIGNS ON PATIENTS GETTING
- blood transfusions
- dopamine infusion
- nitroglycerine infusion
- magnesium sulfate infusion
trivial tasks nursing assisstance do:
- patient feeding; DO NOT LET CNA FEED STROKE PATIENT
-
NEVER delegate something that requires nursing___
judgment; anytime the task needs an RN’s judgement, assessment or problem solving skills.. no matter how trivial
