What are the causes behind hospital malnutrition?

What are the criteria for nutrition support?
Malnourished- BMI < 18.5 OR unintentional weight loss > 10% over 3-6 months
At risk of malnutrition- eaten nothing for 5 days
Poor absorptive capacity
What are the complications of enteral feeding?
Misplaced NGT
Hyperglycaemia
Deranged electrolytes
Ulcers
What are the complications of parenteral nutrition?
Infection at catheter injection site
Pneumothorax
Hyperglycaemia
What is refeeding syndrome?
When you reintroduce carbs to someone that’s starved they may start experiencing arrhythmia, tachycardia = cardiac arrest/sudden death
Coma
Seizures
Wernickes encephalopathy
How do you manage refeeding syndrome?
Provide thiamine 30 mins prior
Start on 10-20 calories a day and slowly increase
Monitor electrolytes
How do you calculate % weight loss
Current/previous weight x100
Is albumin an indicator of malnutrition?
No
Albumin synthesis decreases it can be a predictor but not indicator
