Define hypogonadism
Not enough production of testosterone or sperm
Primary Hypogonadism
Primary: Genetic diseaeses: Klinefelter, Y-chromosome microdeletions, cryptorchidims
OR
Acuired: infectious (mumps, echovirus), radiation, meds, trauma, alcohol
Secondary Hypogonadism
Kallman syndrome, Prader-Willi, stress, malnutrition, excess exercise, hyperprolactinemia, chronic opioid use
Confirm diagnosis of hypogonadism
Low serum testosterone checked in AM and repeated on 2 occasions.
Total serum testosterone
Include SHBG, albumin and free testosterone.
Bioavailable testosterone includes testosterone loosely bound to albumin and free testosterone.
Treatment of hypogonadism
estosterone replacement therapy can be initiated to maintain secondary sexual characteristics, sexual function, and quality of life.
What to check before starting testosterone therapy
