Understand MDD based on DSM5-TR, Diagnostic Feature of MDE and MDD
A
Diagnostic Criteria for MDD
- 5 or more symptoms at least one is either (1) depressed mood or (2) loss of interest or pleasure during the same 2 week period and represent a change from previous functioning.
- Do not include symptoms that are clearly attributable to another medical condition
The Symptoms of Diagnostic Criteria A

- Experience sadness and hopelessness for most of the day nearly every day.(mood may or may not be noticeable to those around them like anger, withdrawn, or tearful.)
- They no longer enjoy activities they used to find pleasurable
- may experience a change in appetite (weight loss/gain)
- may experience sleep disturbances (insomnia/excessive sleeping)
- In some cases, may experience psychomotor agitation (pacing or handwringing) (Unintentional, repetitive movements)(observable by others)
- Feeling of fatigue and exhaustion
- Feeling of worthlessness or inappropriate guilt
- Reduced concentration which make normal functioning difficult
- Recurrent thoughts of deaths/suicidal ideation without a specific plan, or a suicide attempt or a specific plan to commit suicide.

Diagnostic Criteria for MDD
Significant distress or impairment in social, occupational, or other important areas of functioning.
C
Diagnostic Criteria for MDD
- Not attributable to the physiological effects of substance/medication.
- Criteria A-C represent MDE.
D
Diagnostic Criteria for MDD

- At least one major depressive episode is not better explained by schizoaffective disorder and is not superimposed on schizophrenia, schizophreniform disorder, delusional disorder, or other specified and unspecified schizophrenia spectrum and other psychotic disorders.

Diagnostic Criteria for MDD
- There has never been a manic or a hypomanic episodes.
Remember!
Must have at least ONE MDE WITHOUT MANIC and HYPOMANIC EPISODES!
Characteristic of MDE
Persistent and not tied to specific thoughts or preoccupations
Pervasive unhappiness and misery
Self-critical or pessimistic rumination
Low self-esteem (feeling of worthlessness and self-loath)
Thought of deaths which is focus on ending one’s life because of feeling worthless, undeserving of life, unable to cope with pain of depression.
Mild
The symptoms may not interfere significantly with a person’s daily activities, although they may still experience some distress.
Moderate
The symptoms may interfere with a person’s ability to function normally, including difficulty with work, school, or social interactions.
Severe
The symptoms can be debilitating and may even lead to hospitalization, with significant impairment in a person’s ability to function in all areas of life.
