Lymphoma Flashcards
Which lymphoma is most common?
Non Hodgkin-60% > Hodgkin-40%
What is lymphoma?
Neoplastic proliferation of lymphoid cells ->
form mass @ lymph node/extra nodal tissue
What are the basic classifications of lymphoma?
What is the normal B area of lymph node
Small - follicular, mantle, marginal
Cortex
Explain follicular lymphoma.
Treat
14:18 translocation
BCL2 over expressed -> block apoptosis of cells that fail somatic hyper mutation ->
overtime can pick up more mutations =
diffuse LARGE B cell lymphoma
Treat = chemo + rituximab=antiCD20 AB
What two things cause follicle to enlarge?
Infection = follicular hyperplasia
+
Follicular lymphoma
Compare follicular lymphoma and hyperplasia
BAT Mon
BCl2 expression @FL NOT FH
lymph node Architecture disrupt @FL
Tingible Body macrophages NOT present @FL
Monoclonality @FL 20:1 kappa:L chain
️patient gets a neoplastic proliferation immediately adjacent to the follicle. What is it?
Mantle cell lymphoma
11:14 translocation
Over express cyclin D1 -> ⬆️G1->S phase ->
Neoplas prolif
What is marginal zone lymphoma associated with?
What is burkitt lymphoma assoc with?
Forms?
Chronic inflammatory diseases eg
Sjogrens, Hashimoto, Hpylori
EBV!!!! + Assoc with c-myc translocations - nuclear regulator activate multiple genes for growth promotion
African form = jaw involved
Sporadic = ABDOMEN
What is the most common non Hodgkin lymphoma?
Present when?
Diffuse large B cell lymphoma
Enlarging lymph node/extra nodal mass @ adulthood
What are the malignant cells in Hodgkin lymphoma?
What they do??
Diff between non-Hodgkin+Hodgkin?
Reed sternberg cells -> secrete cytokines ->
- draw in inflammatory cells -> form mass
- get B symptoms - fever, weight loss, night sweat
Malignant cells = lymphoid cells vs reed sternberg Mass = lymphoid vs inflammcells/fibrosis Clinical = painless LAD @ late adult vs painless LAD + B symptoms @ young
