Causes of generalized lymphadenopathy
MIAMI
Malignancies (lymphoma, leukemia, metasta)
Infection (brucellosis, bartonellosis, CMV, HIV, EBV, streptococci, rubella, tuberculosis, syphilis)
Autoimmune (SLE, arthritis)
Miscellaneous (Kawasaki doessss, sarcoidosis)
Iatrogenic (serum sicknes, drugs)
EBV symptoms
Malaise, fatigues, Fever, sweats
Sore throat, dysphasia
Anorexia
Nausea, headaches, photophobia
Signs Splenomegaly, hepatomegaly Bradycardia Periorbital edema Palatal enanthem Rhinitis Mucous membrane swelling, reddened throat and tonsils with whitish coating
Complication of EBV
Acute
- Rash
- airway obstruction
- splenic rupture
- lemirre syndrome
Delayed
- chronic active EBV infection
- oral hairy leukopenia
- lymphoproliferative disorders
Kawasaki Disease diagnosis criteria
A- 5days of fever B- plus four or five \+ erythema of lips or oral cavity or cracking of lips \+ Rash on trunk \+ Swelling or erythema of hands or feets \+ red eyes (conjunctivitis) \+ swollen lymph nodes
C- disease cannot be explained by sone other known disease process
KD lab finding
leukocytosis with left shifting in WBC, elevated CRP, ESR, throbocytosis
Elevated ferritin
Platelet counts rise
Normocytic, normochromic anemia
Hyponatremia
Mild to moderate elevation of transminases
Treatment of KD
IV IG 2g/ kg administered as single infusion over 8 to 12 hrs
And aspirin 30 to 50mg/kg daily divided into four doses
PFAPA
Periodic Fever
Aphthous stomatitis
Pharyngitis
Cervical adenopathy
