Lyme disease is caused by
Borrelia burgdorferi
The vector of lyme disease is
Ixodes ticks
Do everyone bitten by the tick gets infected?
No only rarely people are infected due to spirochateocidal activity in serum
Clinical feature stages
Stage 1: spring/summer
Stage 2: several weeks or months later
Stage 3: several months/ years later
Stage 1 clinical fratures
Flu like symptoms
Arthralgia
Skin rash (erythema chronicum migrans)
Stage 2 clinical features
Subacute lymphocytic meningitis
Subacute encephalitis
Cranial nerve involved- facial nerve palsy
Peripheral neuropathy
Bannwarth’s syndrome
Subacute demylinating and axonal sensory/motor neuropathy with severe radiculitis
CSF examination in stage 2
Lymphocytosis
Oligoclonal bands
Increased immunoglobulin
Increases anti burgdorferri antibodies
Stage 3 clinical features
Arthritis
Diffuse CNS involvement
Chronic/subacute encephalitis
Focal brain disease
Psychiatric disease with fatigue and diffuse muscle pain
Diagnosis
IFA
ELISA
PCR
MRI with subcortical white matter lesion
Treatment
Antibiotics
Penicillin
Tetracycline
Erythromycin
