High energy trauma resulting in ligamentous rupture (radioscaphocapitate, scapholunate interosseus, lunotriquetral interosseus)

MOI for lunate/perilunate dislocation
A
- Axial load in a hyperextended and ulnarly deviated position
- Dislocation and/or fracture usually occurs if hyperextension exceeds 100 degrees

Complications of lunate/perilunate dislocation
A
- Median nerve damage
- Skin necrosis 2/2 underlying pressure

Treatment of lunate/perilunate dislocation
A
- Closed reduction followed by IF ASAP
- Open reduction MAY be necessary
