Standing Alignment: Spinal Posture
Slight lumbar extension
Slight Thoracic Flexion
Symmetrical paraspinals
Standing Alignment: Hips
Neutral pelvis
Hips in neutral flexion/extension, neutral rotation, and neutral abduction (feet below hips and shoulders)

Knees: Neutral flexion/extension
Foot/Ankle: Neutral pronation/supination/Abduction/Adduction
Active Forward bending
- Initiation of movement: Posterior pelvic sway and hip flexion
- Rate of lumbar flexion < Rate of hip flexion in the first 50% of the movement
-Quantity: 20 spinal flexion, 70 hip flexion
Return forward bend
Initiation: Hip Extension
- No posterior sway at the trunk at the end of ROM

- Knee should remain aligned approximately over 2nd toe
- Arch should demonstrate slight pronation
Sitting alignment Examiner Position
- Observe with a side view
- Observe patient’s preferred sitting posture with or without a back
- Assess symptoms when exaggerate flexed and extended position
Sitting alignment kinesiological standard
- 90 degrees hip and knee flexion
- Neutral hip abduction/adduction
- Neutral to slightly flexed lumbar spine
Knee extension with dorsiflexion
Lumbar Spine: Stable
Knee: Extends to within 20 degrees of full knee extension
Ankle: Full dorsiflexion without increased symptoms or decreased knee extension
Hip Flexor Length Test Kinesiological Standards
Lumbar Spine: Remains flat
Femur: Neutral
Knee: Flexed to 80 degrees
Hip: Lowered to 10 degree of hip extension
Hip Flexor Test Impairment
- Unable to reach table with the hip and back in neutral alignment indicates short or stiff hip flexor
Supine Alignment Examiner position
- Observe from a side view
- Observe hips and knees extended vs hips and knees flexed
Supine: Hip abduction and lateral rotation with flexed knee Examiner Position
Passive: Assess ROM while monitoring Lumbopelvic region
Active: Monitor ASIS
Supine: Hip abduction and lateral rotation with flexed knee
Passive: No standard, not available ROM
Active: Stable Lumbopelvic region throughout motion
Supine: Unilateral hip and knee flexion Examiner Position
Passive: Monitor Lumbar Spine
Active: Monitor ASIS
