Community Health Nursing Theory: Nightingale’s Environmental Theory
-relationship between patient’s health and the condition that they’re in.
-emphasis on prevention.
-Health is a continuum of care.
Health Belief Model
*predict or explains health behaviors based on people’s beliefs
*preventative health actions are taken b/c person perceive a personal threat to themselves
*emphasis on change at individual level
Milo’s framework for prevention
*compliments Health Belief model but emphasizes change at COMMUNITY level
*behavior changes within a large # of ppl can lead to a social change
Pender’s Health Promotion Model
*doesn’t consider health risks to promote lifestyle changes… don’t care
*defines health as a positive dynamic state, not just the absence of disease
*directed at increasing a patient’s level of well-being.
Transtheoretical of Stages of Change Model
*changes occur over time *Uses 6 stages…
1: precontemplation, I’m unaware that I need to change.
2: contemplation, I need the change, so I weigh the benefits of making that change.
3: preparation, I’m planning to take action.
4: putting the plan into motion, the action stage.
5: maintenance, I’m implementing the actions to continue the behavior. 6: termination. Efforts are no longer needed because I’m consistent

includes a stage where you’re unengaged (where you know there’s an issue but you don’t really care), and then you move into…
2: contemplation.

Primordial Prevention-
reduce RISK FACTORS
playgrounds to promote physical activity, increase tax on cigarettes
Primary Prevention-
PREVENT disease
masking, flu clinics, vaccinations, birthing and newborn classes
Aimed at susceptible populations or individuals
Secondary Prevention-
EARLY DETECTION of disease
Targets: healthy-appearing ppl w/ forms of disease
screenings, paps, colonoscopies, blood pressure screenings
Tertiary Prevention-
REDUCE SEVERITY of disease after diagnosis and reduce potential long-term complications
cardiac rehab for those that have had an MI, diabetic foot care at an affordable rate
Target: symptomatic patients
Quaternary Prevention-
Actions taken to protect patients from medical interventions that are likely to cause more harm than good
Continuum of Care:
Universal prevention-
designed to reach entire groups
Selected prevention-
target individuals or groups with greater risk factors
Indicated prevention-
target individuals who have high probability of developing disease
