What are “irritative” lower urinary tract symptoms?
- frequency- peeing a lot in the daytime
- urgency- insuppressible urge to pee
- nocturia- peeing a lot at night
Interfere with normal STORAGE of the bladder
What are the “obstructive” LUTS?
- weak urine stream
- intermittent
- straining- trouble starting the urine stream
Interfere with normal voiding from the bladder
What are the 7 most common etiologies of LUTS?
- BPH
- prostate cancer
- urethral stones
- urethral stricture
- weak bladder muscles
- bladder stones
- bladder neck sclerosis/contracture

What is the purpose of the prostate?
When does it grow?
What portion of the prostate grows? What is the problem with this?
What does it look like histologically?
It contributes fluid and secretion to semen during ejaculation. It grows under the influence of testosterone.
The transitional zone grows narrowing the lumen of the prostatic urethra with periurethral glandular and stromal hyperplasia
What is the “static” component of LUTS?
the progressive obstruction upon the prostatic urethra by the enlarging glandular and stromal hyperplasia.
What is the “dynamic” component of LUTS?
What drugs can block this?
Bladder neck, prostate, and capsule are innervated by sympathetics (a-1a recepters in high concentration) instead of parasympathetics like the bladder muscles.
Uroselective a blockers (-osin) will decrease tone of the bladder neck and prostate allowing more flow.
What do 5a reductase inhibitors do?
Block testosterone –> DHT which will decrease prostate volume 25%. This relieves obstructive NOT irritative symptoms
What is PSA? What level is very suspicious for prostate cancer?
What is the grey zone? What measurement is taken if the patients PSA is in the grey zone?
PSA - glycoprotein produced by the glandular epithelial cells of the prostate released as free or bound to a1-macroglobulin
Normal PSA 10
What does a urine flow rate recording of less than 10 mean?
It is indicative of an obstruction. Normal flow rate is above 15.
What is residual urine? How much is normal?
What does increased residual urine suggest?
What are the consequences?
Residual urine is the amount that remains in the bladder after attempted complete voiding.
Normal is less than 30cc
Increased is suggestive of:
- obstruction
- weak detrusor muscle
Consequences:
- UTI
- blockage of ureters, kidney–>hydronephrosis
