First line treatment for n/v in pregnancy
Pyridoxine (B6) + Doxylamine (H1 anta)
Med reserved for refractory n/v (2nd trimester)
Glucocorticoids
Eg: methylprednisolone
After giving pyridoxine and doxylamine, pt still experiences n/v. What’s next?
Add Anti-His
Eg: Dimenhydrinate or Meclizine
After adding dimenhydrinate or Meclizine to pregnant pt, but no improvement. What’s next?
D2 antagonist
Eg: Prochlorperazine or Metoclopramide
What is the adverse effect of Prochlorperazine or Metoclopramide?
Extrapyramidal signs –> movement disorder

- Anti-M
- Antacids
- Calcium channel blocker
- Opioid
- 5HT3 anta
3 drugs with anti-M side effects inducing constipation?
- 1st generation anti-H1
- Tricyclic antidepressants
- Typical antipsychotic

Fiber/Bulk-Forming
Eg: Psyllium
2nd step for Constipation tx
Add Saline (Osmotic) Cathartics
How Saline Cathartics increases peristalsis?
Non-absorbable ions –> osmotic retention of intestinal water
Primarily reserved for fecal impaction?
Phosphate enemas
