what is the function of an anti-coagulants?
prevention of new clots and preventing growth of existing clots

heparin or warfarin?
heparin works HELLA fast - within 20mins but its gone in a hurry
warfarin - weaker start - 5 days to start but last longer
can patients take heparin and warfarin at the same time?
why do you think this is?
yes
- because patietns can only receive heparin through IV or SUBQ
- mainly patients are only going to be on this state in the hospital
so we need to slowly start warfarin to let it start working then they can take it at home orally and work just fine
what are the biggest labs associated with heparin and warfarin?
LABS TOO HIGH = PATIENT WILL DIE
LABS TOO LOW = CLOTS WILL GROW
PTT AND INR

what is a low molecular weight heparin?

why would Lovenox be better than normal heparin?
does not require frequent aPTT testing

since vitamin K is the antidote for patients who are on warfarin, we usually recommend these patients to only consume the same amount of green leafy vegetables, don’t take less or more, just normal amount they have been taking
warfarin is a highly protein bound drug, so usually we tell patients to what?
avoid taking other drugs with the medicaiton cause it can increase the effects of warfarin, like aspirin for example
when the patient is sent home with the medication, what are some education we must provide ?
tell their dentist their on this medication
soft toothbrush

don’t smoke
frequent lab test
apply pressure if bleeding for 5-10mins

soybean oil
legumes
fish oilds

if a patient on a heparin drip has a PTT 85 or 100, what is the nursing priority action
if a patient is actively bleeding what does the nurse do?

remember normal range is 46-70
1. stop the heparin drip - notify the HCP
2. prepare antidote ( protamine sulfate )
3. reassess labs in an hour
before you give lovenox, you want to get H&h before hand
tell the doctor if you’re H&H is low, or decreased blood pressure
hold blood thinner if low platelets

- 25inch needle, 5/8inch
- inject at 90 degree
- 2 inches from umbilicus
- BIG NO NO, never aspirate and rub site
