Limb ataxia, intention tremor, loss of balance
Lesion to cerebellar vermis
Truncal ataxia, dysarthria
Lesion to subthalamic nucleus
Contralateral hemiballismus
Lesion to bilateral hippocampi
Inability to make new memories
Lesion to paramedian pontine reticular formation
Eyes look away from lesion
Lesion to frontal eye field
Eyes look toward lesion
