Local exam is never complete without….
examination of draining lymph nodes.
Lymph node examination
Palpate with pads of all four fingertips Examine both sides simultaneously Use steady gentle pressure
Where are the major lymph nodes located
The major lymph node groups are located along the anterior and posterior aspects of the neck and on the underside of the jaw
example cervical nodes- submental, submandibular,preauricular, supraclavicular
what to look for in examination of skin
colour - pallor- iron deficiency, jaundice
pigmentation/localised (pregs) general (liver +renal) texture- lesions, rashes warmth-use back of hand hair distribution turgor moisture
position of thyroid gland
2-3 cm below thyroid cartilage, so basically on cricoid and trachea
Examination of thyroid
Stand behind the patient and place the middle three fingers of both hands along the mid-line of the neck, just below the chin
have the patient drink water
what to remember when examining the abdomen
- quiet room - auscultation
- empty
- examine chest cos similar symptoms
6 p’s
Paleness pain paralysis paraesthesia poikilothermia pulselessness
how does the order go with the 6 p’s
start of with pain then as it progresses paresthesia then eventually parylsis
endarterectomy
removal of plaque from artery
