Hepatitis B after abnormal liver test?

A
  • Start with surface antigen, to find out if infected.
  • Treat when actively replicating, so test for viral DNA
  • Next, determine extent of inflammation or fibrosis of the liver
    • Fibroscan or biopsy, and liver enzymes. Remember that high normal might be inflammed
  • E antigen helpfil to kmow when to stop drugs in treatment. (treament only speeds up natural histpey in verticqllyn infected people.
  • In acute B, we jist support since 95 percent clear it. 1 in 200 will gwt ALF from too robuat response to virus

Cirrhosis

A
  • AST above ALT, need to rule out etoh
  • Get cholestatic, issues moving bile out, get ALP rise
  • Low platelets
    • Portal hypertension, splenic vein backs up, enlarges spleen amd get sequesteration of the plaelets
  • Then younget lover dysfunction and clinical disfunction, INR goes up with bili, albumin goes down
  • ascites varices and HE
