Conjugated Estrogens
Brand: Premarin
Class: Estrogen Hormone
Dosage Forms: Oral Tablet : 0.3 mg, 0.45 mg, 0.625 mg, 0.9 mg, 1.25 mg
Indications:
1. Abnormal vasomotor function (menopause), atrophy of vagina or vulva, postmenopausal osteoporosis prophylaxis, female hypogonadism syndrome : 0.3 mg po daily, continuously or cyclically; adjust dose to individual response
2. Primary ovarian failure : 1.25 mg po daily cyclically (3 wk on, 1 wk off); adjust dose to individual response
Estradiol oral
Brand: Estrace, Various
Class: Estrogen
Dosage Forms: Oral Tablet : 0.5 mg, 1 mg, 2 mg
Indications:
1. Abnormal vasomotor function (moderate to severe), menopause : 1-2 mg po daily for 21 d followed by 7 d off
2. Atrophic vulva or vagina (moderate to severe), menopause : Oral tablet, 1-2 mg po daily in a cyclical pattern (3 wk on, 1 wk off)
3. Breast cancer, metastatic, for palliation only : 10 mg po tid × 3 mo
4. Carcinoma of prostate, advanced, androgen-dependent, for palliation only : 1-2 mg po tid
5. Decreased estrogen level, secondary to hypogonadism, castration, or primary ovarian failure : 1-2 mg po daily
6. Postmenopausal osteoporosis, prophylaxis : 0.5 mg po daily for 23 d followed by 5 d off
Estradiol transdermal patch
Brand: Alora, Climara, Minivelle, Menostar, Vivelle-DOT, Various
Class: Estrogen
Dosage Forms: Transdermal Patch, Once Weekly or Twice Weekly : 0.014 mg/d, 0.025 mg/d, 0.0375 mg/d, 0.05 mg/d, 0.06 mg/d, 0.075 mg/d, 0.1 mg/d
Indications:
1. Abnormal vasomotor function or atrophic vagina or vulva (moderate-severe), menopause : 0.025-0.0375 mg/d patch applied to the skin twice weekly (dose and frequency of patch change dependent on specific product selected); adjust dose as necessary based on response and attempt taper and discontinuation at 3-6 mo intervals
2. Postmenopausal osteoporosis, prophylaxis : 0.014-0.025 mg/d patch applied to the skin once or twice weekly (dose and frequency of patch change dependent on specific product selected); adjust dose as necessary based on response
Ethinyl estradiol and etonogesterel
Brand: NuvaRing
Class: Contraceptive
Dosage Forms: Vaginal Ring : Releases ethinyl estradiol 15 mcg/d and etonogestrel 0.12 mg/d
Indications:
1. Contraception : 1 ring inserted vaginally by patient and remaining continuously for 3 wk, then removed for 1 wk; a new ring is then inserted, regardless whether bleeding has or has not finished
Medroxyprogesterone
Brand: Provera, Depo-Provera, Various
Class: Progestin Hormone
Dosage Forms: Oral Tablet : 2.5 mg, 5 mg, 10 mg
Indications:
1. Abnormal uterine bleeding unrelated to menstrual cycle : 5-10 mg po daily × 5-10 d starting on days 16 or 21 of the menstrual cycle
2. Prevention of estrogen-induced endometrial hyperplasia : 5-10 mg po daily for 12-14 d starting on days 1 or 16 of the menstrual cycle, when estrogen is being administered
3. Secondary physiologic amenorrhea : 5-10 mg po daily × 5-10 d
Oral contraceptive –biphasic
Brand: Kariva, LoSeasonique, Seasonique, Lo Loestrin Fe
Class: Oral Contraceptive
Dosage Forms: Oral Tablet : Biphasic products contain 2 sets of tablets, each phase containing a combination of varying doses of estrogen/progestin agents, or an estrogen agent alone
Indications:
Contraception : 1 tablet po daily beginning either on the 1st Sunday after menstruation begins (“Sunday start”) or on the 1st d of menstruation (“day 1 start”); tablets are taken sequentially, following the arrows marked on the dispenser
Oral contraceptive –monophasic
Brand: Yasmin, Loestrin, Yaz, Ogestrel
Class: Oral Contraceptive
Dosage Forms: Oral Tablet : Monophasic products include tablets that each contains the same fixed ratio of an estrogen and progestin agent
Indications:
Contraception : 1 tablet po daily beginning either on the 1st Sunday after menstruation begins (“Sunday start”) or on the 1st d of menstruation (“day 1 start”); tablets are taken sequentially, following the arrows marked on the dispenser
Oral contraceptive –triphasic
Brand: Tri-Sprintec, TriNessa, Tri-Legest Fe
Class: Oral Contraceptive
Dosage Forms: Oral Tablet : Triphasic products contain 3 sets of tablets, each phase containing a combination of varying doses of estrogen/progestin agents
Indications:
1. Contraception : 1 tablet po daily beginning either on the 1st Sunday after menstruation begins (“Sunday start”) or on the 1st d of menstruation (“day 1 start”); tablets are taken sequentially, following the arrows marked on the dispenser
2. Acne vulgaris, moderate : In females at least 15 y of age who have achieved menarche and are unresponsive to topical antiacne medications, same dosing as for contraception (Ortho Tri-Cyclen and Estrostep Fe are the only products with this FDA-approved indication)
Progesterone
Brand: Prometrium, Various
Class: Progestin Hormone
Dosage Forms: Oral Capsule : 100 mg, 200 mg; Vaginal Gel
Indications:
1. Prevention of estrogen-induced endometrial hyperplasia : 200 mg po daily hs × 12 sequential d per 28-d cycle while conjugated estrogens are administered
2. Secondary physiologic amenorrhea : 400 mg po daily hs × 10 d; Gel, 45 mg (4%) vaginally qod for up 6 doses, if no response, may increase to 90 mg (8% gel) qod for up to 6 doses
3. Assisted reproduction for infertile women : Gel, 90 mg (8%) vaginally once daily, continue 10-12 wk after conception; Insert, 100 mg 2-3 times a day, starting day after oocyte retrieval and continuing 10-12 wk
Risendronate
Brand: Actonel, Atelvia, Various
Class: Bisphosphonate
Dosage Forms: Oral Tablet : 5 mg, 30 mg, 35 mg, 150 mg; Oral Tablet, Delayed Release
Indications:
1. Postmenopausal osteoporosis : Delayed release, 35 mg po once weekly immediately following breakfast; immediate release, 5 mg po daily, 35 mg po once weekly, or 150 mg po once a month; all with supplemental calcium and vitamin D
2. Paget disease : Immediate release, 30 mg po daily for 2 mo
3. Osteoporosis (glucocorticoid induced) prevention and treatment : Immediate release, 5 mg po daily
4. Osteoporosis (male) : Immediate release, 35 mg po once weekly
Raloxifene
Brand: Evista, Various
Class: Selective Estrogen Receptor Modulator
Dosage Forms: Oral Tablet : 60 mg
Indications:
1. Breast cancer, invasive, in postmenopausal women at high risk; prophylaxis : 60 mg po daily
2. Postmenopausal osteoporosis, prevention or treatment : 60 mg po daily
Alendronate
Brand: Fosamax, Binosto, Various
Class: Bisphosphonate
Dosage Forms: Tablet : 5 mg, 10 mg, 35 mg, 40 mg, 70 mg; Solution
Indications:
1. Postmenopausal osteoporosis, prophylaxis : 5 mg po daily or 35 mg po once weekly
2. Postmenopausal osteoporosis, treatment : 10 mg po daily or 70 mg po once weekly
3. Osteoporosis, treatment, male : 10 mg once daily or 70 mg po once weekly
4. Glucocorticoid-induced osteoporosis in those with daily dosage ≥7.5 mg of prednisone (or equivalent) : 5 mg po once daily; a dose of 10 mg po once daily should be used in postmenopausal females who are not receiving estrogen
5. Paget disease : 40 mg po daily for 6 mo
Ibandronate
Brand: Boniva, Various
Class: Bisphosphonate
Dosage Forms: Oral Tablet : 150 mg
Indications:
1. Postmenopausal osteoporosis (treatment) : 150 mg po once monthly
2. Postmenopausal osteoporosis (prophylaxis) : 150 mg po once monthly
