liberation Flashcards
weaning success
is defined absence of ventilatory support 48 hours following the extubation
success rate
out of 2,486 patin the success rate was 68%
weaning failure
defined as failure of a SBT or need for reintubation within 48 hours
Before weaning requirements?
the patient should recovered from the acute phase of the disease leading to mechanical ventilation and be able to assume adequate spontaneous breathing
Paco2 for weaning
paco2 should be between 35-45 mm Hg or around 50 for COPD
Vital capacity for weaning
the vital capacity should be around 10 ml/kilo
and sponatanous tidal volume should be around 5 ml/kilo
Sponatanous frequency
should be less than 35 breath/min and increase in shallow rapid breathing cause increase in deadspace and highly ineffective for gas exchange
time should waited to see change,
3 minutes
minute ventilation
with successful minute ventilation should be less than 10 L/min and acceptable blood gases
for succeful weaningfor oxygenation
pao2 should less than 60 mm Hg and greater than 90% and Fio2 less 40%
Pao2/fio2 ration for weaning
should be equal of greater than 150 mm Hg
Qs/Qt ration
should be less than 20% for successful weaning
a-a difference
should be less than 350 mm Hg on 100%
Maximum inspiratory pressure
for successful weaning should be at least -30 cm h20
static compliance should be
greater 30 mL/cm H20
