What are examples of liver screening tests?
Virology - checking for Hep B and C virus, CMV and Epstein-Barr
Immunology - checking for anti smooth muscle antibodies and antimitochondrial antibodies
Iron studies
Caeruloplasmin
History specific tests - hep E virus, hep A virus and paracetamol
What change in LFTs is seen in a hepatitis pattern?
Rise in ALT and AST
What are come common conditions causing a hepatitis pattern?
Autoimmune hepatitis
Hep B
Hep C
NAFLD
What effect does autoimmune hepatitis have on liver screening tests?
Positive anti smooth muscle antibodies
Elevated IgG
What are some of the risk factors for Hep C?
IVDU
Unprotected sex
Tattoos
What effect does Hep C have on liver screening tests?
Positive HCV (duh!)
What comes under the umbrella term of NAFLD?
Simple steatosis (just fat deposits) NASH (inflammation)
What are some of the conditions causing a cholestatic pattern?
Gallstones
Obstructing tumours
Primary biliary cholangitis
Primary sclerosing cholangitis
How does a cholestatic pattern present on LFTs?
ALP and bilirubin elevation
ALT is mildly elevated/normal
What is the classic presentation of primary biliary cholangitis?
An old woman who is incredibly tired and itchy with a degree of jaundice
What effect does primary biliary cholangitis have on liver screening tests?
Positive antimitochondrial antibodies
What causes a mixed pattern of hepatitis and cholestasis?
Alcohol
Obstructing/parenchymal tumours
