-lewy bodies are confined to the substantia nigra

Dementia with lewy body
A
- lewy bodies are in the substantia nigra but they are also in the temporal lobe, cingulate gyrus and frontal lobes
- they may also be found in the dorsal motor nucleus of the vagus

Cortical Lewy bodies
A
-less conspicious, less eosinophilic and lack clear halo compared to those in the substantia nigra

-there is no simple correlation between the number of Lewy bodies and cognitive decline

How to idenitify Lewy Bodies
A
- antibody to protease ubiquitin can be used
- staining with antibody alpha-synuclein helps identify lewy bodies
- DLB is one of the various degenerative synucleopathies

Microvacuolation
A
- some patients with DLB show microvacuolation of the cerebral cortex predominantly in the medial temporal regions
- this can mimic prion disease
