Flat, non-raised

Ex: nevus, freckles, amalgam tattoo, melanoma
Patch
Flat, non-raised
Larger than 1 cm
Different in color, texture, or both from surroundings
Ex: melanoplakia, port wine stain, hematoma
Petechiae
Pinpoint red spots, up to 2mm, usually in groups
Purpura
Bruise of about 1 cm or less
Ecchymoses
Bruise greater than 1 cm
Plaque
Flat top
Solid
Raised
Larger than 1 cm
Ex: keratosis, leukoplakia, lichen planus, hairy leukoplakia, linea alba
Papule
Elevated, solid, superficial, any color
Less than 1 cm
Pedunculated or sessile based
Ex: papilloma, verruca vulgaris, condyloma acuminatum
Nodule
Raised, flat, or submerge
Solid with a dimension of depth
Any color
Less than 1 cm
Ex: lipoma, traumatic neuroma, neurofibromastosis
Tumor
Raised
Solid mass with a dimension of depth
Any color
Greater than 1 cm
Ex: squamous cell carcinoma, ranula, mucoepidermoid carcinoma
Vesicle
Clear, fluid-filled elevation
Lymph or serum
In epidermis
Less than 1 cm
Ruptures leaving an ulcer or crust
Ex: herpes, chickenpox, shingles
Bulla
Vesicle larger than 1 cm
Ex: pemphigus, pemphigoid, burns, erythma multiform
Pustule
Raised
Filled with purulent exudate
Less than 1 cm
Ex: parulis pericoronitis, periodontal abscess
Erosion
Slightly depressed
Moist
Surface epithelium denuded
Also called abrasion
Ex: erosive lichen planus, pemphigus, herpangina, herpes simplex, allergic reactions
Ulcer
Depressed deeper than erosion
Total loss of epithelial layer
Ex: aphthous ulcers, chickenpox, shingles, trauma squamous cell carcinoma, chemotherapy
Fissure
Linear crack in the epidermis
Ex: angular chelitis, fissured tongue
Scar
Increased area of collagen fibers in connective tissue
Follows healing
Appears white or pale
