lesion attributes Flashcards
describing lesion attributes
“patient appears to present with NUMBER SHAPE TEXTURE COLOR TYPE SIZE.
CONFIGURATION DISTRIBUTION LOCATION.
a AMOUNT amount of CONSISTENCY COLOR ODOR exudate is observable”

exudate
number lesion =
single
multiple
actual
size lesion =
measured in mm or cm
color lesion =
red (erythema)
black
white
yellow
shape lesion =
round
annular (ring shaped)
irregular
texture lesion =
scale (eczema look)
crusted
smooth
firm
fluctuant
type lesion =
macule
papule
vesicle
patch
ulcer
pustule
bulla
fissure
nodule
plaque
define macule
- less than 01 cm
- flat, non palpable (cannot be felt)
- represent change in color, not raises or depressed compared to the skin surface
EX: FRECKLES FLAT MOLES TATTOOS
define papule
- circumscribed solid elevation up to 01cm in diameter on skin
- solid or cystic raised spot
define vesicle
- fluid inside up to 05cm in diameter (clear fluid) (blister)
- ex: anthrax
define patch
- flat discoloration
- greater than 01cm
- large area of color change
- smooth surface
define ulcer
- open sore on skin
- reduction in blood flow
- open wound
- sore on the skin / mucous membrane accompanied by the disintegration of tissue
define pustule
DIFF THAN VESICLE
- pus filled sores
- inside has purulent (pus discharge) material
- common on face, chest, back
- infectious disease, bacteria, viruses, fungi, parasites
define bulla
- greater than 05mm
- large vesicle
- large blisters on the skin that are filled with clear fluid
- caused by infection / inflammation of the skin
define fissure
- split in the skin
- bleeding due to dryness
- abnormal keratinization / exfoliation
define nodule
- similar look to papule / vesicle
- to many cells aggregated in one place. feels like a ball of something but it’s fixed (won’t move, tender, non tender)
define plaque
- circumscribed elevated platoed like greater than 01cm
- solid lesion
- raised red patches colored with silvery white buildup dead skin cells or scale
configuration
grouped
clustered
distribution
generalized (spread out)
localized (one area)
location
superior
posterior
anterior
proximal
distal
inferior
frontal , transverse , sagittal
what information does exudate give us
amount, consistency, color
exudate amounts
- no visible
- scant (moist tissue)
- small (visibly moist tissue)
- moderate (saturated tissue)
- large (saturated tissue draining freely expressed / GUSHING)
exudate consistency
- watery
- thin
- runny
- thick
- sticky
exudate color
red
black
white
yellow
clear
opaque
cloudy
skin color descriptions
- pale (low circulation)
- jaundiced
- mottled (pattern of redness / paleness due to body temp)
- cyanotic (blue = no oxygen, central reached organs body is trying to protect)
- flushed
define maceration
- skin in contact with moisture for TOO LONG
what happens to the skin when it’s been macerated
- light in color
- wrinkly
- wet
- soggy to the touch
what is a woods lamp
BLACK LIGHT
- distinguish hypopigmentation
(decrease) from depigmentation (loss)
- darken room and 4 to 5 inches from affected area
what conditions will fluoresce with woods lamp?
TINEA VERSICOLOR
- fluoresce pale white - yellow
what conditions will not fluoresce with woods lamp?
TINEA …
cruris
corporis
pedis
what is wound culture
SPECIMEN
- small sample taken from the body to represent the nature of the whole
what is the goal of the tissue specimen
provide lab with a sample biologically representative of the substance collected
what to determine for wound culture
collecting
WOUND SPECIMEN
OR
TISSUE / STS SPECIMEN
have to’s for a physical examination
