- Schedule 1 - little or non therapeutic value, only used for research
- Schedule 2- their use is strictly controlled
- Schedule 3 - high potential for abuse/misuse, but with therapeutic value
- Schedule 4 - Some potential or abuse/misuse and therapeutic value
- Schedules 5 - Some potential for abuse/misuse and therapeutic value

- Signature of the prescriber
