- Feedback from staff & clients, processes (e.g. water cups, eco paper, zero waste to landfill, recycling)
- Feedback from clients, processes (feedback forms in discharge letter, in reception area, anonymous feedback forms, client leaflets, on website, transparent complaints procedure), Observation

What notifications do you need to report to CQC?
Death of a person using the service
Serious injury to a person using the service
Allegations of abuse
Police involvement in an issue
Events that stop a service running safely and properly (e.g. significant staff shortages, loss of key equipment, or building issues).
Absence of a registered manager for 28 days or more (and then their return).
