A

Who can sign on behalf of a minor:
1) Legal guardianship trumps anything!
2) Attorney for child in foster care
3) If attorney is not an answer option, LCSW would be next in line.
4) if mental health is court ordered, foster parent can consent
5) If a guardian brings child in and doesn’t know where parents are located care affidavit should be signed
6) If court has issued minute order for LCSW or foster parent to sign
&) Minor 12 and older if determined to be mature enough to do so and there is reason to not include parents
