What is it?
When cardiac output becomes inadequate for the body’s requirements
What are the 2 classes of cause?
Systolic and Diastolic
What happens in systolic?
Inability of the ventricle to contract normally causing a reduced ejection fraction <40%
What is the cause of systolic? (3)
Ischaemic Heart Disease, Myocardial Infarction, Cardiomyopathy
What happens in diastolic?
Inability of the ventricles to relax and fill causing increased filling pressures, ejection fraction is >50%
What is the cause of diastolic failure? (4)
Constrictive pericarditis, Tamponade, Restrictive Cardiomyopathy, Hypertension
Which is most common?
Systolic and Diastolic usually co-exist
What happens when it mixes with right ventricular failure?
You get congestive heart failure
Is it common?
Yes, prevalence is 40%
Who does it affect?
People over 70, generally males
Symptoms (11)
Dyspnoea, Poor exercise tolerance, fatigue, orthopnoea, paroxysmal nocturnal dyspnoea, nocturnal cough, pink sputum, nocturia, wheeze, weight loss, muscle wasting
Signs (4)
Cyanosis, hypotension, displaced apex beat, murmurs
What bloods would you do?
FBC, BNP, U&E
Other investigations:
ECG, Chest X-ray, ECHO
What is low output failure?
Cardiac output is low and fails to increase on exertion
