What are some exceptions to electronic vs written prescription writing?
Practitioner and dispenser cannot be the same entity.
Sometimes, prescriptions cannot be transmitted electronically.
Waiver due to economic hardship, technology limitations.
FDA requires special elements in the prescription.
Research drug
Adverse impact if not given written ASAP.
Hospice care/nursing home resident.
Best interest of patient, must be documented!!!
What is the general format of a prescription?
Patient’s name + DOB
RX: Lisinopril 10mg
SIG: 1 tab PO daily for x days
DISP: #30 tablets
Refills: 3 (three)
Signature/date/info
How many patient identifiers do I need in a prescription at minimum?
2!
Usually name + DOB
What goes into the RX section of a prescription?
Drug name + dosage (units) + formulation (i.e. ER, LA, XR, SR, etc.)
i.e. Lisinopril 10mg (has no ER or anything formulation)
How do I write a prescription if I want the brand version?
Write brand name
Include a statement such as “no generics” or “brand name only” or “DAW” (Dispense as written) or check the box saying no generics.
What goes into the SIG row?
How to take the med.
When to take the med
Special directions.
i.e. 1 tablet PO daily for 7 days
OR
1 tablet PRN every 4-6 hours for R knee pain.
What does form mean for a medication?
How the medication is packaged, such as a tablet, capsule, cream, gel, aqueous, patch, etc…
What does route mean for a medication?
How the medication will be administered, such as orally, injection into muscle, rectal, topical, etc…
What are some common route abbreviations for prescription writing?
PO (by mouth)
PR (per rectum)
IM (intramuscular)
IV (intravenous)
ID (intradermal)
IN (intranasal)
TP (topical)
SL (sublingual)
BUCC (buccal)
IP (intraperitoneal)
What are some frequencies for medications?
daily (DO NOT USE QD)
every other day (NO OTHER ABBREVIATION)
BID (twice a day)
TID (three times a day)
QID (four times a day) during waking hours
QHS (at bedtime)
Q4h (every 4 hours)
Q6h (every 6 hours) usually for stricter regimens.
QWK (every week)
What does prn mean?
pro re nata, aka as needed.
What can go into special instructions?
Indications, max dose per day, to take with food or not, location of application (r knee).
What do you put before the number in DISP?
Advised to spell out the number after writing it, but not required.

What can limit refill amounts?
Laws, insurance, and # dispensed.
What are some DO NOTs for prescription writing?
Do not use U for unit or IU for international unit.

Lack of leading zero.
