Professional:

  • Assess physical function limitations and disabilities related to the neurological, musculoskeletal and cardiopulmonary systems
  • Determine a treatment plan
  • Apply treatment in order to obtain optimal functional performance

Reserved:

  • Assess neuromusculoskeletal function (physical function limitation/disability)
  • Functional assessment
  • Introduce instrument or finger in the human body beyond the labia majora or anal margin (perineal reeducation)
  • Introduce instrument in the human body in and beyond the pharynx or the nasal vestibule (chest physio)
  • Use invasive forms of energy (electrotherapy)
  • Provide treatment for wounds

how often will you be inspected by PIC in Quebec and what does it entail?

A

ON-SITE ASSESSMENT:

  • Inspector goes to the workplace of the member
  • Inspector performs a thorough analysis of indicators
  • Inspector proceeds with a competency evaluation based on PIC’s normative framework

Self evaluation

The member is required to self-evaluate one of the records he/she is submitting to inspection. The grid used for this self- evaluation is the same used by the inspector… This is to ensure transparency.

Reference books for evaluation of appropriate and pertinent items by members, inspectors and investigators

Evaluation Grids are designed for inspectors and investigators to assess the record-keeping of the members (present, absent, incomplete or non applicable items entered)

WHAT ARE THE PURPOSES OF MAINTAINING COMPLETE AND UP-TO-DATE CHARTING FOR EVERY PATIENT? (3)

  1. Ensure quality of services

–Helps clinical reasoning/organize thoughts

–Records therapist’s interventions within the context (professional setting, values of the pt and PT, influence of other member of the team,….) and allow evaluation (pertinence of the services)

–Assists in decision making (3rd party, other professionals)

  1. Continuity of services (communication tool)

–colleagues, team, third party payers,….

  1. Witness of practice

–defense tool…proof in court or professional audit

  • What is not written is considered not done, your word against the plaintiff

read over the charting basics

  • Chart as soon as possible
  • Write clearly and legibly
  • Use accepted abbreviations –Don’t create your own abbreviation system
  • Signature (name & title PT or PT student) –Initial evaluation: full name
  • Correction of mistakes –Draw a line on the error, date it and initial it

common mistakes:

O is not an analysis, Specify active or passive ROM, Identify the affected body part, Compare affected side to healthy one, Specify the right or the left side, Measured or observed, Don’t overuse «N» (for sensation, tonus), Not written: considered not done

be precise:

“patient doing well” –Compared to what? Where is your evidence? What precisely have you observed?

“patient ambulating today” –How? With aid or not? How far? What speed? How is his balance? Gait pattern?

“patient uncooperative” –It is better to say « Patient refuses to walk » as it is a fact and not a judgment.

ethical considerations:

  • Be attentive to your patient’s dignity (facts that could stigmatize patients, not all facts should be written -even if true, might not be pertinent, if the information is questionable, needs to be stated…..)
  • Confidentiality…. Fax, internet, chart sharing
  • Who will have access to the chart? (future employers, insurance companies,… )

Performs an audit if a professional is suspected to have contravened the OPPQ Code of Ethics or the Professional Code.
