Lec 6: Principles of Rehab Med Flashcards
Goals of neurologic rehabilitation
Reduce impairment
Facilitate recovery
Allow adaptation to persisting disability
The objective findings of MMT and ROM cannot be elicited in the presence of:
a. Atrophy
b. Flaccidity
c. Spasticity
d. Examiner applied resistance
C. spasticity
Objective testing cannot always be done in patients with: Cognitive impairment Pain Deconditioning Contracture/deformity Spasticity Mass Joint Instability Malingering (Hyper/hypo mobility) Muscle Girth
Includes the non-rehab members in the group, which
include NGOs, employers, halfway homes,
educational institutions, other clinical departments,
etc
Multidisciplinary Team Approach
Psychiatrist brings together rehab physicians
(neurologists, neurosurgeons) and allied health
members (physical/occupational/speech therapist,
nurse, orthotist, prosthetist, vocational counselor
psychologist)
Interdisciplinary Team Approach
any psychological, physiological, anatomical structure
loss at the organ tissue/illness level
Impairment
Restriction of ability to perform within normal
parameters
disability
Disadvantage in societal role performance, on an
environmental and social level
Handicap
Impairment, disability or handicap? cervical spinal cord injury
Impairment
Impairment, disability or handicap? difficulty in swallowing and speech
Disability
Impairment, disability or handicap? loss of income as call center employee
Handicap
What is the strength of quadriceps based on MMT? a patient who is seated can extend knee against resistance
grade of 4 or 5 depending on amount of resistance
What is the strength of gluteus medius based on MMT? full hip abduction while supine
grade of 2
What is the level of dependence? Inability to physically nor verbally assist in task
Dependent
What is the level of dependence? Physical or supervisory assist of one or more persons to perform task safely and
within functional period of time
Assisted independence
What is the level of dependence? Completes task safely and within functional period of time with or without aids
Independent
What are the 3 mechanisms of neuroplasticity?
- Enhancement of denervation supersensitivity
- Guidance of axonal sprouting
- Facilitation of unmasking of latent synapses in head injuries
T/F. Delayed rehabilitation appears to have a strong
relationship to improve functional outcome at
hospital discharge and follow-up
F. The earlier, the better
What should the intensity be in rehab?
Passive exercises –> gradual increase depending on response –> Resistive exercises
T/F. Mobility can be achieved only by walking
F. Mobility can be achieved with various implements (ex.
quad cane, single cane)
What device is used to facilitate physiologic upright position?
Tilt table
Keeps ankle stable so that there is no foot drop. Increases the speed of walking for stroke patients
Ankle foot orthosis
What is the management for spasticity?
Intrathecal Baclofen Therapy, Orthopedic, Neurosurgical
What are the causes of pressure sore occurence? A. pressure B. shear/friction C. excessive exposure to moisture D. nutrition imbalances E. A, B, C F. A, C, D G. AOTA
G
Pneumonia prevention for those in recumbent position
in a long time
PULMOPHYSIOTHERAPY
