Conditions for an emergency supply at prescriber request?
- Appropriate prescriber
- Emergency
- A prescription can be obtained in 72 hours
- Directions
- NOT for controlled drugs except phenobarbital (schedule 3)
- Record kept in POM register
What needs to be in the POM register for emergency supply?
- Patient details (name and address)
- Medicine details (name and quantity)
- Prescriber details (name and address)
- Date supplied
- Date on prescription
- Date prescription received
What prescribers are allowed on an emergency supply at request of prescriber?
Doctors, dentists, supplementary prescribers, all independent prescribers, practitioner nurse prescribers and EEA/Swiss Prescribers.
HCP’s outside EEA/Switzerland = not recognised as relevant in the UK
Emergency supply at request of patient conditions:
- Emergency/ Immediate need
- Interview
- Satisfied it has been used before - SCR (with permission), old bottles etc.
- Dose confirmation
- Not for CD’s except phenobarbital
- Length of treatment - no more than 5 days for phenobarbital or 30 days for POM –> unless abx, pill, inhaler
When does an emergency supply at request of a patient need to go in the register?
on the day of supply or next day
What needs to do in the POM register for emergency supply at request of a patient?
- Date POM supplied
- Medicine details - dose, quantity etc
- Patient details - including address
- Information on nature of Rx
How long is a CD 2,3,4 script valid for?
28 days
Maximum quantity for 2,3,4 CD scripts?
30 days
What needs to be on a dental CD script?
For dental treatment only
When is a standardised CD form required?
Only for s2 and s3 CD’s
Does a sch3 drug need to be recorded in the CD register?
No - 1 and 2 only
Conditions for CD register?
Must be: chronological, in indelible ink and unaltered
The CD’s that need to be kept in safe custody are
Sch 1
Sch 2 - except some liquid preps
Some Sch 3 - temazepam, buprenorphine, common exemptions = phenobarbital, midazolam, tramadol,
Does sativex need to be in a register and what schedule is it?
Yes and 4 (stored in fridge)
CD requisition requirements?
Signature, name, address and profession of recipient, total quantity of drug and purpose of requisition
When is an approved form (cd requisition) not required?
Request of hospice/prison
Can EEA prescribers prescribe Sch 2 or 3 drugs?
No - 4 and 5 only
What CD’S can a podiatrist prescriber NOT prescribe

Can a physio authorise an emergency supply for phenobarbital?
No
Can physio and podiatrist IP’s Rx unlicensed medicine?
No - only off label
Can schools purchase adrenaline without a prescription?
Yes - in small quantities and if on an occasional basis
What records need to be kept and made in the pharmacy?
Signed order (keep for 2 years) Enter in POM register: Date POM supplied Name, quantity and strength of medicine Name and address, trade, business and profession Purpose for which it was sold
Maximum day supply and validity of Isotretinoin rx’ions?
7 days validity
30 days supply - only can be dispensed more if patient isn’t under PPP
prescription requirements for Vet RX?
- Name, address, telephone number, qualification and signature of prescriber (If CD, registration number must be there)
- Name and address of owner
- Identification and species of animal
- Date - valid for 6 months and 28 days for CD2, 3, 4
- Name, quantity, dose and admin instructions
- Any necessary warnings
- Use under cascade if necessary
- For sch 2 and 3 - a declaration that item has been prescribed for an animal or herd under the care of a veterinarian
- If it is repeatable - the number of times it can be repeated needs to be on there
Are standardised forms required for Vet sch2 and 3 drugs?
No - just the statement item has been prescribed for an animal or herd under the care of a veterinarian
Is the physical presence of a pharmacist required to supply vet medicines?
Yes unless transaction has been authorised by pharmacist
Do vet prescriptions need to be kept?
Yes for 5 years
Do vet prescription records need to be kept?
Yes for POMV and VPS
What needs to go in the vet script record?
Name of medicine Date of receipt or supply Batch number Quantity Name and address of supplier or recipient
Label requirements for Vet product?
Name and address of animal owner name of prescribing vet surgeon Name and address of pharmacy Identification and species of animal Date of supply The words keep out of reach of children and for animal treatment only Expiry date of product Name/description of product Dosage and admin instructions Any necessary warnings Any applicable withdrawal period
Max amount of paracetamol and aspirin that can be sold?
100 non effervescent (includes soluble/disperible)
no limit on effervescent
Max amount of codeine/dihydrocodeine that can be sold?
32 dose units
If more, Rx needed and needs to state can cause addiction and for three day use only
