$1,000 per individual license, $2,000 for DLSM license, and $3,000 for salon license.
How do you handle a cut or abrasion?
Stop service, put on gloves, clean, and disinfect tools
How do you disinfect a pedicure tub?
Immediately after each service. Drain and remove visible debris, disinfect, scrub tub with detergent, water rinse and drain. Replace filter parts, epa disinfectant, drain and rinse with clean water.
How are dirty towels stored?
Stored in a covered container labeled “used”

How often do you have to mix a new disinfectant?
Daily or when you can see see visible debris in the solution.
How do you disinfect sheers or razors between each use?
Doesn’t required to be washed, but must be wipe down hairs and debris and spray with hospital level disinfectant that is EPA registered.
What do you do before disinfecting?
Clean off any visible debris and remove disposable parts then wash item with soap, water, rinse, and dry.
How often do you disinfect towel warmer?
Daily with disinfectant wipe or spray.
If you took extra product, can you put it back in the container?
No, the product is now contaminated.

If a salon doesn’t have a ventilation system, what must be done?
Thermostat fan must be continuously on during business hours.
After finished with hair service, when and where must you sweep hair?
From station, chair, floor, and anywhere else right after service.
Are animal allowed in the salon?
No, only service animal are allowed.
What are single use, disposable, item?
Toe separators, pumice stone or bar, foam flip flops waxing sticks, nail files and buffers; anything porous can only be used once then discarded after each service.
How do you take product out of a jar?
Must use a disposable spatula or disinfected applicator.
Can you reuse a client’s lashes on another client?
No, the lashes must be discarded.
When must lash extensions stand, holders palettes and trays be cleaned and disinfected?
Before each use with each client.
What’s the requirement for cleaning hands?
Must wash hands with soap and water and dry with clean cloth or paper towel. Alcohol based hand sanitizer can be used if it’s at least 60% alcohol. No gloves or hand wipes.
What do you do if you need more time to fix a violation?
Provide board with written explanation of the reason for the delay, the steps you will take to fix it and projected date it will be fixed.

Must have state ID. They can inspect whenever they deem necessary. Everyone must comply with the inspection. Inspections happen at least once per year.
What does ‘good repair’ means?
Clean, free from holes, fraying, tears, rips, fully operational for intended purpose.
