Lateral Ankle Sprain (Hamilton) Flashcards
Inman’s analogies:
the medial knee is analogous to the
lateral ankle

the talus is part of a cone: which part of the cone is medial?
the apex

the talus is part of a cone: which part of the cone is lateral?
the base

the deltoid ligament corresponds with which part of the cone
the deltoid is medial, corresponds to the apex of the cone

what are the ligaments of the lateral ankle?
- posterior talofibular ligament
- calcaneofibular ligament
- anterior talofibular ligament

in what order do the ligaments of the ankle tear?
- ATLF
- CFL

What is a grade I ankle sprain?
There are many classifications of ankle sprains
- Partial tear of ATFL
- drawer test
- T tilt test
O’Donoghue
What is a grade II ankle sprain?
- Tear ATFL
- Intact CFL
- drawer test
- T tilt test

How long does it take for a high ankle sprain to heal?
twice as long as ATF sprain

What is a grade III sprain?
- complete torn of ATFL and CFL
- instability (triplanar)
- drawer test
- T tilt test
Drawer sign?
- done in 15 deg PF
- Can be graded I, II, III

calcaneal anterior process fracture

- is the attachment of EHB and EDB
- may happen during sprain of the foot
- conservative Tx
Shepherd’s fracture
- Fracture of the lateral tubercle of the posterior process of the talus
- typically resulting from ankle inversion, forced plantar flexion, or direct compression injury
- the posterior talofibular ligament avulses the tubercle

Which other problems can come with lateral ankle sprains?
- Shepherd’s fracture (PTFL)
- Calcaneal anterior process fracture (EHB & EDB)
- Talar dome lesions
Acute lateral ankle sprain treatment options
- RICE
- Grade I: early rehab, WBAT, air CAM
- Grade II: air CAM
- Grade II: Sx rarely used
