Later Stroke Management Flashcards
What kind of cortical remodelling occurs after stroke?
Dendritic sprouting, synaptic remodelling.
If a stroke occurred in the right hemisphere, what visual field would have defects?
The left visual field.
What is neglect as applied to a stroke?
A RIGHT HEMISPHERE stroke causes visual/sensory defects in the left side.
What is Agnosia?
The inability to access recognise things and interpret sensations. - May be able to detect things using other sensory modalities.
What is Prosopagnosia?
Inability to recognise faces.
How long is a post-stroke driving ban for minimum?
1 month
What is Dyspraxia?
The loss of ability to conceptualise, plan and execute complex motor sequences.
What defect causes Dyspraxia?
Loss of cortical pathways - not muscle weakness
Where may anatomical lesions for Dyspraxia be?
> Left inferior parietal lobe
> Supplementary motor area
Spasticity of which muscles causes a spastic semi paretic gait?
> Ankle plantar flexors
> Knee flexors
Why does having a spastic semi paretic gait increase the risk of falls?
> Ankle is weak
> Unable to dorsiflex; hard on uneven surfaces
What is spasticity?
Form of paralysis w/ hyper excitable tendon reflexes. May be accompanying tendon/soft tissue shortening.
What are the consequences of spasticity?
Impaired balance, pain, poor hygiene in flexures, lost dexterity.
What is the treatment/management for spasticity?
Manage - Physiotherapy.
Treatment - Botulism toxin injection or Baclofen (GABA agonist).
