A 2 year old child with no immunization history presents with high fever, sore throat. He is drooling with dysphagia, his voice is muffled and he has an inspiratory stridor. What does he have? What organism is usually responsible? What is he at increased risk of?
He has acute epiglottitis which is inflammation of the epiglottis. H influenzae type b is the most common cause, especially in non immunized children. Risk of airway obstruction.
A patient presents with a hoarse, barking cough and inspiratory stridor. What does he have? Which part of his airway is affected? WHich organism usually causes this?
Laryngotracheobronchitis (Croup). Inflammation of the upper airway. Parainfluenza virus.
A singer who is on the last leg of his tour presents to the voice clinic with hoarsness which goes away with rest. What does he have? Which side is usually involved? What type of tissue is involved?
He has vocal cord nodule (Singer’s nodule) that arises on the true vocal cord. Due to excessive vocal cord use, usually bilateral. Composed of degenerative (myxoid) connective tissue.

Laryngeal Papilloma which is a benign tumor of the vocal cord. Due to HPV 6 and 11. Papillomas are multiple in children though.
A 56 year old patient with a past medical history of alcohol use and tobacco presents with hoarsness. He has a cough and stridor as well. What does he have?> Where does it come from?
Laryngeal carcinoma which is a squamous cell carcinoma usually arising from the epithelial lining of the vocal cord. Can rarely arise from a laryngeal papilloma. Risk factors are alcohol and tobacco.
