Q

What are some causes of elevated prolactin?

A
  • pregnancy
  • prolactinoma
  • pituitary stalk damage (trauma, surgery, pituitary tumors)
  • DA blockers (haloperidol, risperidone)
  • primary hypothyroidism

Q

What test should be done if acromegaly or gigantism are suspected?

A
  • check GH and IGF_1 levels (IGF-1 is preferred)

- oral glucose tolerance test: give glucose and GH should be decreased. if GH remains elevated–> problem –> imaging

6
Q

What is the medical treatment for acromegaly?

A
  • Pegvisomant (GH receptor antagonist). binds to the GH receptor but does not activate the JAK2-STAT signaling or IGF-1 secretion
  • DA agonists –>

Q

What can happen to blood sugar with elevated GH?

A

GH acts to increase gluconeogenesis, decrease skeletal mm glucose use and decrease adipose glucose uptake

  • IGF-1 acts as insulin to decrease blood glucose
  • GH effects> IGF-1 ==> elevated blood glucose
